Please read these terms carefully before using our services. By booking an appointment with US Mobile Lab, you agree to the following terms and conditions.
US Mobile Lab provides mobile phlebotomy and specimen collection services. We are not a diagnostic laboratory; we do not analyze specimens on-site. All specimens are transported to a CLIA-certified laboratory partner (Quest Diagnostics, Labcorp, or your physician’s designated lab) for analysis.
Our services include blood draws, urine specimen collection, wound culture collection, and related specimen handling. Results are delivered directly to your ordering physician. We do not interpret test results or provide medical advice.
Patients must provide a valid physician’s lab order (requisition) at the time of service. Appointments without a valid requisition may be cancelled without refund.
Patients are responsible for ensuring their insurance information is current and accurate. Lab testing fees charged by the laboratory are separate from our service fee and are the patient’s responsibility.
Patients must be present or have an authorized adult present at the time of collection. For pediatric patients, a parent or legal guardian must be present throughout the procedure.
US Mobile Lab charges a flat service fee of $70 per visit. This fee covers travel, specimen collection, and transport to the laboratory. This fee is separate from any laboratory analysis fees charged by Quest, Labcorp, or your insurance.
Payment is due at the time of service. We accept major credit cards, debit cards, HSA/FSA cards, and cash. We do not currently bill insurance for our mobile service fee, though your laboratory fees may be billed through your insurance by the lab.
Cancellations made less than 2 hours before the scheduled appointment may be subject to a $25 cancellation fee.
Appointments may be booked online 24/7. Same-day appointments are subject to availability. We reserve the right to reschedule appointments due to staff availability or unforeseen circumstances.
To cancel or reschedule, please contact us at least 2 hours before your appointment. Late cancellations (less than 2 hours notice) may incur a $25 fee. No-shows will be charged the full $70 service fee.
In the event that we are unable to complete a draw (e.g., difficult access, patient uncooperative), the service fee may still apply at our discretion.
US Mobile Lab is committed to protecting your health information in accordance with the Health Insurance Portability and Accountability Act (HIPAA). We collect only the minimum necessary information to provide our services.
Your protected health information (PHI) will not be shared with third parties except as necessary to fulfill your lab order (i.e., with your ordering physician and the laboratory), or as required by law.
You have the right to request access to your PHI, request corrections, and file a complaint with the U.S. Department of Health and Human Services if you believe your privacy rights have been violated. Contact us at info@usmobilelab.com for privacy-related requests.
US Mobile Lab’s liability is limited to the cost of the service provided. We are not liable for laboratory errors, delayed results, or medical decisions made based on test results.
We carry professional liability insurance for all phlebotomy services. In the event of a specimen collection error attributable to our staff, we will re-collect at no additional charge when clinically appropriate.
By booking our services, patients acknowledge the inherent risks of any medical procedure, including bruising, hematoma, fainting, and infection at the venipuncture site, which are rare but possible outcomes.
US Mobile Lab reserves the right to update these terms at any time. Changes will be posted on this page with an updated effective date. Continued use of our services after changes constitutes acceptance of the revised terms.

These terms are governed by the laws of the State of Michigan. Any disputes shall be resolved in the courts of Oakland County, Michigan.
