At US Mobile Lab we value clarity, fairness, and simplicity in all billing matters. Below you’ll find how our private-pay mobile phlebotomy services and insurance processes work — so you always know what to expect.
💳 1. Private-Pay Specimen Collection
Specimen collection services are private pay.
This means the phlebotomy visit itself is not billed to insurance.
The mobile blood draw and collection fee are paid directly to Us Mobile Lab by the patient or facility.
Our standard mobile collection fee is $70, covering travel to your home, office, or facility within our Michigan service area.
There are no hidden surcharges — just one transparent, flat rate for the visit.
🧪 2. Insurance for Laboratory Testing
Insurance is collected only for the testing laboratory to bill for the actual laboratory tests performed.
Once we collect your specimen, it’s delivered to your testing laboratory (Quest. Labcorp, ln-Network Laboratory).
That laboratory will process the test and bill your insurance directly for the testing portion — not for the specimen collection visit.
Any copay, deductible, or uncovered portion of testing is determined by your insurance provider and billed by the testing lab.
💰 3. Payment Timing
Payments that have not been prepaid will be due at the time of service.
For private-pay clients, payment is required at or before your appointment.
We accept all major credit and debit cards (Visa, MasterCard, Discover, Amex), as well as cash, Zelle, or ACH for corporate/facility clients.
Receipts are automatically provided for all payments.
🕒 4. Invoicing & Facility Accounts
- Facility partners and corporate clients may be billed monthly with approved credit terms (net 30 days).
Late payments beyond 30 days may incur a 5% late fee.
📅 5. Cancellation & Rescheduling
We understand plans change. To avoid fees, please cancel or reschedule at least 24 hours before your scheduled appointment.
- Same-day cancellations or no-shows: 50% of the service fee ($35) may apply.
If our phlebotomist arrives and cannot collect due to missing paperwork, access issues, or nonpayment, the full service fee may still apply.

Oakland • Macomb • Lapeer • Genesee • Wayne Counties
🔒 7. CLIA-Certified & Transparent Billing
US Mobile Lab is fully insured and the labs are CLIA or COLA accredited (Quest, Labcorp, Etc.).
We are committed to transparent billing — no surprise charges, ever.
